Personal Information & Records

Your personal information is housed in myHR. This information includes your:

  • Online Directory Information such as name and title
  • Mailing address, emails, phone numbers, and emergency contacts
  • Voluntary self-identification of disability, gender, ethnicity, race, and veteran status

Legal Name Changes

Former and current employees must submit a Personal Data Form and a copy of their updated Social Security Card showing the new legal name change.

  1. Complete the Personal Data form.
  2. Scan or copy your Social Security card that shows your updated name.
  3. Submit the form to Human Resources:
    1. Email the completed form with the card to askHR@northwestern.edu OR
    2. Mail or drop off the requested items to Human Resources.  See address at Directions & Parking: Human Resources.

Students and faculty must also submit the legal name change separately to your Registrar’s Office for recording in CAESAR.  Please see further information on the Registrar's website.  

Legal name changes in myHR are sent to our benefit providers on a weekly basis.  Once the legal name change has been updated in myHR, no further steps are needed. 

Online Directory Information

Most employees are able to modify their display names by following these steps:  

  1. Log into myHR (multi-factor authentication required)
  2. Click the “Personal Details” tile and select "Online Directory" on the left and follow the steps outlined

Note: Temporary employees do not appear in the online directory and therefore are unable to change their display name.  

Address Changes

CURRENT EMPLOYEES

Active/currently paid employees with access to myHR should update their address information online. 

  1. Log into myHR (multi-factor authentication required)
  2. Click the “Personal Details” tile and select "Addresses" on the left side to update your address.
    1. IMPORTANT: Updating the address in myHR DOES NOT impact state tax withholdings on the paycheck.  Employees who are working outside of Illinois must notify Human Resources to ensure that the appropriate tax forms are completed.  Visit our Tax Information webpage for more details.  

FORMER EMPLOYEES

  1. Complete the Personal Data form to update your address on file. 
  2. Submit the form to Human Resources:
    1. Email the completed form with the card to askHR@northwestern.edu OR
    2. Mail or drop off the requested items to Human Resources.  See address at Directions & Parking: Human Resources.
