Deadlines for Immunization Requirements

Full-time (3 or more classes) and half-time (2 classes) students at Northwestern are required to meet immunization requirements. Part-time (1 class) students are not required to fulfill these requirements. This includes submitting the Student Immunization Form and proof of immunizations as required by the State of Illinois. Distance learners that will never attend on-campus classes in Evanston or Chicago are not required to submit immunization records. Students who are currently attending interim online classes as an alternative to physically attending in-the-classroom on the Evanston or Chicago campus are not classified as distance learners; therefore, required to submit immunization records.

The Student Immunization Form and proof of immunizations should be submitted to the Evanston Campus Northwestern University Student Health Service by the applicable deadline below.

STUDENTS ARRIVING FROM OTHER COUNTRIES in need of a TB test and/or a chest X-Ray as determined by the TB self-screening in the Student Immunization Form have 30 days after the start of classes to complete the process without incurring a penalty. Please call Northwestern Medicine Student Health Service-Evanston at 847.491.8100 to schedule an appointment after arriving on campus.

Penalties for Non-Compliance

Make sure to submit the Student Immunization Form and proof of immunizations by the applicable deadline above. This allows Northwestern University staff time to process your Student Immunization Form and immunization documents and notify you of any deficiencies. Furthermore, it provides you with the time to rectify any deficiencies prior to arriving on campus.

Students who fail to submit their Student Immunization Form and record of immunizations, or fail to rectify ALL immunization deficiencies by 30 days after the start of classes will be barred from future class registration (including adding or changing classes) until compliant and will be assessed a non-refundable $100 late fee. PENALTIES ARE NOT ASSESSED PRIOR TO THE START OF CLASSES.
