Certificate Requests
When a formal agreement or contract requires a Certificate of Insurance to fulfill contractual obligations, submit a certificate of insurance request with a minimum of 5-7 business days prior notice. All requests for certificates of insurance require the following information. Please refrain from submitting a certificate of insurance request unless all the relevant information requested below is available.
- Signed Agreement or Contract
- Certificate holder information (requesting third-party name, address, and contact information)
- Purpose/type of activity
- Location(s) in which coverage is required
- Dates for which coverage is required (specific or a range)
- Insurance coverages and limits contractually required to carry
- Itemized List of Equipment, if requesting equipment/property coverage
- Whether liquor is being served, if request is for an event
