Immunization Requirements for Non-Healthcare Students

FULL-TIME (3 classes) and HALF-TIME (2 classes) students are required by Illinois State law (Illinois Administrative Code 694.100) and University regulations to provide proof of immunizations, in English. PART-TIME (1 class) students are not required to fulfill these requirements. Proof of immunizations along with the Admission Health Record must be submitted to the Evanston Campus Health Service prior to entering a full-time or half-time academic program. Acceptable forms of documentation include at least one of the following:

  • Recording of vaccination dates by a licensed healthcare professional (e.g. physician, nurse, etc.) in "PART II: Required Immunizations" of the Admission Health Record.
  • Copies of immunization records from a healthcare professional, previous school, immigration documents, or other official record.
  • Copy of a lab report confirming immunity is acceptable in place of vaccinations for Measles, Mumps, and Rubella.

Immunizations required by State of Illinois and Northwestern:

  • Measles (Rubeola): Two vaccinations of Measles or MMR administered after first birthday, and after 1/1/1968 OR physician’s documentation of illness and date of diagnosis.
  • Mumps (Parotitis): One vaccination of Mumps or MMR administered after first birthday, and after 1/1/1968 OR physician’s documentation of illness and date of diagnosis.
  • Rubella (German Measles): One vaccination of Rubella or MMR administered after first birthday, and after 1/1/1969. Date of illness not accepted for Rubella.
  • Tetanus/Diphtheria Booster (DT): One booster vaccination administered within 10 years of entrance into Northwestern.
  • Tetanus/Diphtheria Series (DPT, DTP, DTaP): Students that are attending Northwestern on a Student Visa are required to supply at least 2 dates of vaccination from their Tetanus/Diphtheria primary series (e.g. DTP, TDaP) in addition to the booster.
  • Tuberculosis (TB) Self-Screening: Non-healthcare students must complete the TB self-screening questionnaire (PART III of the Admission Health Record).

STUDENTS ARRIVING FROM OTHER COUNTRIES in need of a TB test and/or a chest X-Ray as determined by the TB self-screening in the Admission Health Record have until 20 days after the start of classes to complete the process with incurring a penalty. Recommended for all students (not required)

  • Meningococcal (Meningitis): One vaccination of meningococcal vaccine (or Menactra).
