Privacy and Confidentiality

The privacy of your health information and confidentiality are key concerns to the staff of the Health Service. Depending on the specifics of any patient visit to the Health Service or other interaction via telephone or other form of communication, we strive to ensure only those staff who have a specific need have access to your health information.

Certain medical procedures may require completion of a consent form which will explain the aspects of a particular patient care process and your rights related to the health information resulting from your care at the Health Service.

Since June 2007, the Health Service has transitioned to use of an Electronic Health Record system which provides many improvements to the overall management of health information, and in particular, the privacy and confidentiality of this data. If you have specific questions related to your health information, please contact the  Health Information Management Services at or call 847.491.2203.
