Patients may schedule appointments for allergy shots only after all forms have been completed and received at the Northwestern Medicine Student Health Service.
- Allergy extract has been received via overnight shipping from allergist's office to Student Health Service,Attention Allergy RN, only via FedEx or UPS. Postal Service cannot deliver overnight to this location.
- Shot schedule and vials have been reviewed and approved by Student Health Service consulting allergist
- Students will be notified when above requirements are completed and they are approved to schedule an appointment
All forms (Referring Allergist Agreement, Student Agreement, Allergen Immunotherapy Order, and Administration of Allergy Immunotherapy Instructions) must be updated every academic year beginning September 1.
Instructions to shipping allergy extract to the Student Health Service
- Extract must be shipped directly from the allergist's office (not by patient or family) overnight to arrive at NM Student Health Service between 8:30 a.m.- 5:00 p.m. Monday-Friday and not on University holidays.
- Vials must be shipped overnight by UPS or FedEx. Extracts cannot be shipped via USPS. Vials shipped by USPS are sent to a campus mail service prior to delivery to NM Student Health Service and will not be received by the office the day after mailing.
- Extract which is not received the day after it is shipped from the allergist's office cannot be used for patient therapy.
- The Student Health Service follows guidelines from Summary Statement 84 in the 2011 Allergen Immunotherapy: A Practice Parameter Third Update. It says,: "extract manufacturers conduct stability studies with standardized extracts that expose them to various shipping conditions. It is the responsibility of each supplier or manufacturer to ship extracts under validated conditions that are shown not to adversely affect the product's potency or safety." This included round ship shipment to Phoenix in the summer, San Antonio in August and Alaska during the winter. The Student Health Service does not ship vials with refrigerant or cold packs.
- The Student Health Service follows guidelines for long term extract storage per above reference (Summary Statement #83) at 4 degrees C to 8 degrees C.
Instructions for Allergy Immunotherapy Patients
The Student Health Service Allergy Clinic policies and procedures have been revised to be consistent with current American Association of Allergy, Asthma & and Immunology (AAAAI) recommendations and guidelines for allergy immunotherapy.
The Student Health Service does not sign forms from other allergy offices. The guidelines followed at The Student Health Service are listed in the Referring Allergist Agreement (PDF).
We require three forms:
- A “Student Agreement for Allergy Immunotherapy Administration” (PDF) form completed by all new and returning allergy and immunotherapy patients prior to receiving allergy shots at the Health Service (updated agreement required annually on September 1).
- A “Referring Allergist Agreement” (PDF) form signed by your referring allergist. Your allergist will need to review both our Allergy Clinic Policy and Procedures and our protocol for “Physician Management of Anaphylaxis and Systemic Reactions” before signing the agreement (updated agreement required annually on September 1).
- Administration of Allergy Immunotherapy Instructions (PDF)
You should print these documents out and provide them to your allergist to review and sign. We will not be able to administer your allergy shots until both signed forms are received.
