In accordance with the Affordable Care Act (ACA), beginning 1/1/15, all new independent contractors must be pre-approved by Human Resources before a vendor code is requested. An Independent Contractor Questionnaire (ICQ) is required as a new first step. ICQ is not needed only in cases of a) an existing vendor receiving additional payment, b) contractors that have already been approved on a prior ICQ, and c) companies.
