In accordance with the Affordable Care Act, beginning 1/1/15, all new independent contracts must be pre-approved by HR before a vendor code is requested. An Independent Contractor Questionaire (ICQ) is required as a new first step. ICQ is not needed only in cases of a) an existing vendor receiving additional payment, b) contractors that have already been approved on a prior ICQ, and c) companies.
