Request an Outreach Program
Thank you for your interest in partnering with Chicago CAPS! If you would like to request an outreach program or presentation from Chicago CAPS, please complete the Chicago Outreach Request Form below. This form should be submitted for all requests, including requests for CAPS presentations at new student orientations.
- Please submit this request form at least 3 weeks prior to your preferred program date.
- Please indicate the steps you will take to guarantee that at least 10 participants will be in attendance.
- Due to limited staff resources and availability, particularly during periods when CAPS is experiencing peak clinical demands, we may not be able to meet all requests.
- A CAPS staff member will review your submitted request form and will be in contact with you within 5 business days.
