Camp/Clinic/Lesson Forms
Initial Submission for Camp/Clinic/Lesson Approval
- The camp/clinic/lesson approval form must be submitted via ARMS at least one month prior to a camp/clinic/lesson.
- Please be sure to attach any printed materials that will be used to promote camps and clinics.

Final Participation
- A final participant list must be submitted within fourteen (14) days of the conclusion of the camp/clinic/lesson. This list must include all participants, grade in school, type of camper, payments, discounts, and an explanation for any irregularity in payment.

Staff & Compensation
- A final staff & compensation list must be submitted within fourteen (14) days of the conclusion of the camp/clinic. This list must include position/title, total compensation, and any discrepancy in pay.
- All camp/clinic employees, including any volunteers, must have completed the required DCFS training and background check requirements as outlined in the camp manual.
SUBMIT YOUR FINAL STAFF & COMPENSATION FORM
Payment Form
- The payment form should be completed immediately following any camp, clinic, lesson, or club team practice
- The form should be submitted along with a check written out to Northwestern University
