Cash Disbursement Form

This form is used when giving student-athletes meal money. The amount of money that each student-athlete will receive should be written on the form before each athlete signs their name.
Meal money can only be provided when a team meal is not provided, and there is a maximum amount per meal that can be given ($10 for breakfast, $10 for lunch, and $20 for dinner).
Student-athletes cannot receive both meal money and have a team meal provided to them for the same meal.
This signed form serves as a receipt for an Expense Report.
