Requesting/Replacing Your Card

If your card is lost or stolen, you must notify the Wildcard Office immediately. If your Wildcard has been activated to act as your U.S. Bank ATM/debit card, notify U.S. Bank at 800-US-BANKS (872-2657).

You will be charged a $25 replacement fee for a new card ($50 for biometric). If it has been damaged, it must be turned into the Wildcard Office to avoid a $25 replacement fee.

Requesting Your Card


In alignment with the university's plan to keep density in common areas minimal, students, faculty, and staff that are required to be on campus can book an appointment with a Wildcard Office that is convenient to them to stop by and pick-up a new, renewal, or replacement ID (cards will be printed the morning on the day of the appointment).

Please follow the below process (if your appointment is for a renewal, uploading a new photo is optional).

Upload and submit your photo for your Wildcard (in-person photos are not offered at this time). Book an appointment to pick-up your Wildcard. Ensure your Wildcard is programmed for facility access.




The below form is for requesting an individual Wildcard for mailing (please do not forget to submit your photo, requests will not be processed without a photo on record). The form will authenticate your account by asking you to login with your NetID/password.

**Wildcard Mailings are postponed and will resume 10/15/2022.**


 Note: You must log in with your NetID and password. Students and benefits eligible faculty and staff only.

Please do not submit this request if you are…

  • An Undergraduate First Year/Transfer Student. Additional information on distribution for first year/transfer students can be found here.
  • A student living on campus. An appointment should be made to pick-up your Wildcard (please refer to the above).
  • A Part-time/Temporary Employees or Contractor. Information on PT/Temp and Contractor requests can be found here.


If you request your ID card be mailed to you, DO NOT also request an appointment to pick it up in person. If you need to change your request for whatever reason, please contact

  • Mailings will be processed on Wednesdays weekly. Mailings will not be processed over university holiday/break periods (the link will be disabled as we approach university break periods).

  • At this time, we are not able to process mailings for international addresses.
