Requesting/Replacing Your Card

If your card is lost or stolen, you must notify the Wildcard Office immediately. If your Wildcard has been activated to act as your U.S. Bank ATM/debit card, notify U.S. Bank at 800-US-BANKS (872-2657).

You will be charged a $25 replacement fee for a new card ($50 for biometric). If it has been damaged, it must be turned into the Wildcard Office to avoid a $25 replacement fee.

Requesting Your Card


Students, faculty, and staff can book an appointment with a Wildcard Office that is convenient to them to stop by and pick-up a new, renewal, or replacement ID (cards will be printed the morning on the day of the appointment).

Please follow the below process (if your appointment is for a renewal, uploading a new photo is optional).
