Disability Accommodations

Under the Reasonable Accommodation Policy, OCR provides reasonable accommodations to faculty and staff (including student workers).

Visitors, job applicants, and students can learn more about accommodations and accessibility resources.

Steps to Requesting a Disability Accommodation

1. Requestor completes the accommodation request form.

The first step in requesting a disability accommodation is to fill out a an accommodation request form (PDF version).

2. Requestor submits current medical note to OCR

After filling out an accommodation request form, Requestors then submit a current medical note. The medical note must be from a licensed health care provider.

Requestors also have the option of signing and submitting an authorization form to give OCR permission to contact their provider’s office to request the needed information. 

3. OCR reviews request

OCR reviews the request and contacts Requestor with any clarifications or further information needed.

4. OCR makes an initial determination regarding a reasonable accommodation

After reviewing the request, OCR will make an initial determiantion based on the materials submitted.

5. OCR contacts the department

OCR will contact the relevant department to have a confidential conversation and answer any questions a supervisor may have. OCR will not disclose any medical information.

6. OCR makes a final determination

OCR's final determination includes considering alternative reasonable accommodations if necessary. The office will communicate the decision to the Requestor, their department and, if applicable, other university stakeholders
