You must complete this form and receive an approval from the MSA staff. Failure to comply with any of the regulations listed below may result in your group losing room reservation privileges.
Things to Remember:

Approval of your room reservation will be sent via email within 24 hours (or by the next business day if you submit a room reservation request on Friday, weekend, and/or holiday).

Submitting the online Room Reservation Form does not automatically confirm your reservation.

Parties, amplified music, candle and incense burning, smoking, and alcohol are prohibited.

In compliance with University Risk Management, the house occupancy rate is 49 people at the Black House and the MCC.

All leftover food and trash must be thrown away properly.

Any large pieces of trash must be placed in the dumpsters behind the building.

Furniture may be moved between rooms but please return them to their original location.

Clean up the room after your meeting/event and leave it in a presentable manner.

Grace Periods:

For all meetings and events, there is a grace period of 15 minutes.

If you do not show up after 15 minutes, your space may be used by any person or group needing the space.
Changes to Your Reservation:

If you need to make changes to your reservation, please use the online Room Reservation Form.

However, if changes are for a reservation that will occur within 24 hours, please call Jerre Michelin (847-491-3610 for the Black House) or Claire Kelly (847-467-6200 for the MCC).

The MSA staff reserves the right to modify room reservations in order to accommodate multiple groups using the building during the same time. You will be contacted if such changes need to be made.
