Fabricated Equipment
What is Fabricated Equipment?
Fabricated
equipment is defined as an item of equipment that is built or
assembled from individual parts by a PI and/or other sponsored project
personnel, an internal shop, or an external shop.
Procedures
Fabricated equipment should be identified prior to the acquisition
of parts or services related to the fabrication. This identification
can be made, initially, in the sponsored project proposal budget
or, later, using an OSR-2 form. In addition to identifying the fabrication
in the proposal budget or OSR-2 form, the Fabricated Equipment Form
and the Fabricated Equipment Questionnaire must be completed to
provide specific information necessary to ensure that the circumstances
constitute a fabrication and later to track the fabricated item.

Equipment Questionnaire before you complete the fabricated equipment
form to help ensure the proper classification of equipment.

Submit Forms
Submit the completed Fabricated Equipment Form, the Fabricated
Equipment Questionnaire and the OSR-2 Form to OSR by: e-mail to
