Summary of Immunization Requirements
FULL-TIME (3 or more classes) and HALF-TIME (2 classes) students at Northwestern are required to meet immunization requirements. PART-TIME (1 class) students are not required to fulfill these requirements. This includes submitting the Student Immunization Form and proof of immunizations as required by the State of Illinois. Distance learners that will never attend on-campus classes in Evanston or Chicago are not required to submit immunization records. Students who are currently attending interim online classes as an alternative to physically attending in-the-classroom on the Evanston or Chicago campus are not classified as distance learners; therefore, required to submit immunization records.
To ensure that you complete the appropriate form and supply the correct immunizations, select the category below that best describes your program:
- Undergraduate, graduate, continuing and professional (SPS) studies, non-degree programs that are not healthcare programs including the MS in Communication (MSC program)
- Graduate or professional-level healthcare program which includes: Communication Sciences & Disorders, Genetic Counseling, and Prosthetic Orthotics. (Medical, Physician Assistant and Physical Therapy students do not complete this form)
- Medical students only
- Physician Assistant students only
- Physical Therapy students only
NOTE: Penalties for not fulfilling the immunization requirements include a hold on subsequent registration and a non-refundable $100 late fee. PENALTIES ARE NEVER ASSESSED PRIOR TO THE START OF CLASSES. See the Requirement Deadlines page for more information.

COVID-19 VACCINATION REQUIREMENT
Proof of Northwestern-required COVID-19 vaccination will be submitted separately from the immunizations required by the State of Illinois, via the COVID-19 Vaccination Form. Please refer to the email sent to students on June 7, 2021 for details. Additional information may be found here.
