Authorization for Release of Protected Health Information
Tax Documents
1095 Form - a health insurance tax form which reports the type of coverage you had and the period of coverage for the prior year. This form is used to verify on your tax return that you have had at least minimum qualifying health insurance coverage.
To obtain your 1095 form online from your Aetna Navigator Account please visit Aetna's website, and follow the instructions.

Please reference Plan # 46543 when visiting an EyeMed Vision Care network*. You get a discount off eye-care items that don’t need a prescription, too. Like eyeglass chains, contact lens cases and cleaners, nonprescription eyeglasses and more.

Dental Discounts: Vital Savings on Dental
Vital Savings on Dental is a dental discount program that helps you and your dependents save.- Savings range from 15-50 percent on dental services
- Applies to general dentistry and more
- There are no claims to file.
Because Vital Savings by Aetna is supported by the Aetna Dental Access® network, you have over 209,000** available dental practice locations to choose from nationwide. Each dentist is credentialed and re-credentialed on an ongoing basis.
This is optional with your Aetna Student Health plan for an additional $25 fee.
You may enroll in the Vital Savings Program at ANY time during the plan year. Discounts are available the day payment is made thru the end of the current plan year.

Beginning Right Maternity Program
The Beginning Right Maternity program can give you extra support:- Tools and resources to help you have a healthy pregnancy.
- Learn about prenatal care, labor and delivery, newborn and baby care, and more
- Get helpful information for your baby’s father or your partner.
- You may also get follow-up calls after delivery to make sure you and your baby are OK.
