Extended Care Plan Meetings

Extended Care Plan Meetings are in-person and may be scheduled only after completing an Initial Care Plan Phone Meeting.

Why do I need to schedule an Extended Care Plan Meeting?

The primary aim of the Extended Care Plan Meeting is to obtain a more detailed understanding of the student’s current reasons for seeking mental health services and their mental health history. We will work with you to determine what next steps and resources would be the best match for you. This may include connecting to services at CAPS or connecting to referrals in the community.

How long will the meetings be?

Students should anticipate spending 60-65 minutes at CAPS for an Extended Care Plan Meeting. Students will spend approximately 15 minutes completing initial paperwork and then will meet individually with a CAPS therapist for approximately 45-50 minutes.

What if I am late?

If a student arrives more than 15 minutes late for the scheduled appointment time, we may ask you to reschedule the appointment so you have sufficient time to complete all paperwork and attend the Extended Care Plan Meeting.

Can I cancel or reschedule?

If you need to cancel or reschedule your Extended Care Plan Meeting, please call us at (847) 491-2151 with at least 24-hours notice.

What do I need to bring?

Please bring your Wildcard and a copy of your insurance card to your Extended Care Plan Meeting.

We ask that you bring a copy of your insurance card so that if you are recommended to connect with a community provider, CAPS would then be able to provide you with referrals that take your specific insurance plan.
