Begin Coverage

To enter a piece of equipment into the program:
  1. Download and complete the Equipment Inventory Change Form word document icon (.doc) for the desired item.
  2. Fax it to Specialty Underwriters (414-281-1111) to Jacki Frank's attention.
  3. You will receive a quote for the item within five days.

Cancel Coverage

Contracts automatically renew annually, so contracts must be canceled if you wish to cease coverage.

To remove equipment from this program:

E-mail both Jacki Frank of Specialty Underwriters and Ellen Barnes of NU University Services. Provide the following information about the equipment in your e-mail:

  • tag number
  • model number
  • serial number
