To enter a piece of equipment into the program:- Download and complete the Equipment Inventory Change Form
(.doc) for the desired item. - Fax it to Specialty Underwriters (414-216-0343) to Earl Bednar's attention.
- You will receive a quote for the item within five days.

Cancel Coverage
Contracts automatically renew annually, so contracts must be canceled if you wish to cease coverage.
To remove equipment from this program:
E-mail both Earl Bednar of Specialty Underwriters and Ellen Barnes of NU University Services. Provide the following information about the equipment in your e-mail:
- tag number
- model number
- serial number
