Complete a Campus Security Authority Report

Campus Security Authorities are University personnel who have been designated by the Clery Act as mandated reporters for any crime defined by the Clery Act.  If you are unsure of your role, please contact the Compliance, Audit, and Advisory Services.

If you ARE NOT a Campus Security Authority (CSA)

Please use the General Concern Report to report any crime or criminal activity of which you have been made aware.

If you ARE a Campus Security Authority

Please use the CSA Crime Report Form to report any qualifying crime or criminal activity of which you have been made aware.  Please note, this form does not guarantee follow up with the community members involved in these incidents.  If response is needed from the Division of Student Affairs, please also fill out the appropriate form on our How To Report a Concern page.
