Open Enrollment extended to Dec. 5

Open Enrollment for the 2004 benefit programs has been extended to Dec. 5.
To change benefit selections, or to re-enroll in the Flexible Spending Account (FSA) Plan, forms must be received by the Benefits Division or by the Chicago Campus Human Resources Office by 5 p.m. Friday, Dec. 5.

Among the changes are a new HMO, eligibility of over the counter medications for reimbursement from the FSA health care account, new prescription drug benefit manager, and new spouse and dependent term life insurance plans.

Employees may obtain information on 2004 changes through the Open Enrollment Web site as well as the Benefits Division Open Enrollment booklet, which will be sent to all benefits-eligible faculty and staff.
