The deadline for submitting Flexible Spending Account (FSA) claims for calendar year 2002 services is tomorrow (Feb. 28).

Participants have until Feb. 28 to submit reimbursement claim forms to the Benefits Division for services received during the prior calendar year.

Any account balance as of March 1 will be forfeited.

FSA Health Care Account participants may submit a reimbursement claim and receive reimbursement for eligible expenses of up to the full annual pledge amount less any year-to-date reimbursement.

Completed claim forms should be submitted attaching the appropriate Explanation of Benefit statements (EOB), receipts, invoices or other documents indicating provider, patient, amount and date(s) of service.

FSA Dependent Care Account participants may submit a reimbursement claim form and receive reimbursement for eligible expenses of up to the total amount they had contributed to the Account less any year-to-date reimbursement. Completed claim forms should be submitted with the appropriate receipts, invoices or other documents indicating name of the dependent and provider, provider’s social security number or Federal Tax I.D. number (FEIN) and amount.

Claim forms may be obtained by download from the Benefits Division Web site at www.northwestern.edu/hr/benefits/forms-literature/claims/.

Completed claim forms with attachments must be received by the Benefits Division (720 University Place, Evanston, Ill. 60208) or the Chicago campus Human Resources office (710 N. Lake Shore Drive, Chicago, 60611) by 5 p.m. Friday (Feb. 28).
