DEAN CERTIFICATION
Some professional and graduate schools, particularly law schools, as well as state boards and federal agencies require applicants to submit a “Dean’s Certification” form that provides information on academic and disciplinary issues that students may have encountered as an undergraduate.

Student/Alumni Responsibilities
Students/alumni can drop off, fax, or mail any document requiring academic or judicial verification to:

In order for us to process the form, you must have the following:
- Your Dean’s Certification form(s) signed and dated.
- If your form does not explicitly authorize Northwestern University to release information, then you will need to also complete a Consent for Release of Personal Information/Education Records form. This can be found on the Registrar's webpage at: http://www.registrar.northwestern.edu/PDFs/gen_consnt.pdf.
- There is no need to fill out a separate Consent Form for each school to which you are applying; on the form, simply give Northwestern University permission to release your Judicial Record to all of the schools to which you are applying, for the purposes of your law school applications. (if you have questions about this, feel free to call for more information).

- Your current contact information in case we need to contact you.
- A stamped, self-addressed envelope for each Dean’s Certification you would like completed and mailed.
Please note: Although every effort is made for a quick turn-around on these forms, completion of Dean’s Certification may take up to 5-7 working days to complete, so please plan accordingly.
Process
Students/alumni of the university may start the Dean’s Certification process by contacting the Office of Judicial Affairs. In order to relay an accurate picture of the student’s academic and disciplinary history, the Office of Judicial Affairs will communicate with the student’s academic college. In both cases, only suspension or exclusion will be reported to external third parties. For the disclosure policy of a specific academic school, please contact that school directly.
The Office of Judicial Affairs' official policy on disclosures to external third parties is as follows:
Disciplinary matters resolved by the Division of Student Affairs that result in suspension and/or exclusion will be reported to external third parties, as required or permitted by law. If a student has any record, a statement explaining our reporting policy will also accompany the request.
The statement that will accompany a request is as follows:
Northwestern University’s record retention policy for disciplinary matters adjudicated by the Division of Student Affairs is as follows. Student disciplinary files (paper and electronic formats) are maintained by the Office of Judicial Affairs. Disciplinary matters that result in suspension and/or exclusion from the University are kept permanently. All other disciplinary files are destroyed upon the student’s graduation from Northwestern.
