What Vision plan does Northwestern University offer?

UnitedHealthcare Vision is the stand alone vision plan that Northwestern University offers to all benefit eligible employees.

Do I have vision coverage through my medical plan?

Yes, each medical plan offers its own vision discount plan. You can find additional information located on our Vision site on each of the vision plans offered.

If I have a medical problem with my eyes, will my vision plan cover this?

No, please consult your medical care physician for these concerns.

Can I purchase the standalone vision plan in addition to the coverage included in my medical plan?

Yes, but the two programs cannot be used simultaneously.

If I enroll in vision coverage and have vision discounts through my medical plan, will I have to pay anything out of pocket?

Double coverage is not a guarantee of 100% payment. You should review the plan provisions of UHC Vision vs. what is in your medical plan to use both to your advantage.

What is my unique ID number with UnitedHealthcare Vision?

Your unique ID is your employee ID number, located on the front of your wildcard. If you are asked for a 9-digit value, use your employee ID with two zeros in front of it. For example, if your employee ID is 1234567, then you would use 001234567

How do I submit claims?

•  If using a network provider you do not need to submit claims.

•  If using an out-of-network provider, obtain a vision claim form from either UnitedHealthcare Vision or the vision discount plan associated with your medical coverage, and submit it to them for reimbursement.
