Below are forms for the Tuition plan. For the forms pertaining to more than one benefit plan, please refer to Human Resources Benefits Forms Page.

NOTE: Historically, every December when invoices for the winter term of the next calendar year are produced, Student Accounts receives numerous phone calls and e-mail messages about the invoices for Educational Assistance and Reduced Tuition study not being correct. This is because a new tuition benefit application has not been submitted for receiving tuition benefits for the next calendar year and the benefit has not been applied to the account. Some of the parents/students have suggested that they should be notified reminding them to submit a new application. To facilitate the generation of correct winter term invoices issued in mid to late December, individuals should submit a new tuition benefit application for courses taken during the upcoming winter term by December 1st. The absolute deadline for submitting a tuition benefit application to the Benefits Division is December 31st for courses taken during that calendar year.

Educational Assistance - Employee Study at Northwestern University
2009 Educational Assistance Claim - Undergraduate	Use this form to receive the employee discount on undergraduate classes or classes through the School of Continuing Studies for the 2009 calendar year.
2010 Educational Assistance Claim - Undergraduate	Use this form to receive the employee discount on undergraduate classes or classes through the School of Continuing Studies for the 2010 calendar year.
2009 Educational Assistance Claim - Graduate	Use this form to receive the employee discount on graduate classes or classes through the School of Continuing Studies for the 2009 calendar year.
2010 Educational Assistance Claim - Graduate	Use this form to receive the employee discount on graduate classes or classes through the School of Continuing Studies for the 2010 calendar year.
2009 Educational Assistance Claim - Satellite Offices	Use this form to receive the employee discount if your customary work place is located more than 200 miles from Chicago

Reduced Tuition Claims – Spouse or Dependent Child Undergraduate Study at Northwestern University
2009 Reduced Tuition Claim	For full-time employees with dependent children or spouse, who wish to receive tuition assistance for classes attended at NU during the 2009 calendar year.
2010 Reduced Tuition Claim	For full-time employees with dependent children or spouse, who wish to receive tuition assistance for classes attended at NU during the 2010 calendar year.
Reduced Tuition Claim -Contributed Service Medical School Faculty Member	Submit the Reduced Tuition Claim form above, along with documentation from your Department Chairman that you participate in the equivalent of 78 hours per year in scheduled teaching to:   Office of Deans Administration Rubloff 12th Floor 420 E.
