Below are forms for the Tuition plan. For the forms pertaining to more than one benefit plan, please refer to Human Resources Benefits Forms Page.

NOTE: Historically, every December when invoices for the winter term of the next calendar year are produced, Student Accounts receives numerous phone calls and e-mail messages about the invoices for Educational Assistance and Reduced Tuition study not being correct. This is because a new tuition benefit application has not been submitted for receiving tuition benefits for the next calendar year and the benefit has not been applied to the account. Some of the parents/students have suggested that they should be notified reminding them to submit a new application. To facilitate the generation of correct winter term invoices issued in mid to late December, individuals are reminded in mid November to submit a new tuition benefit application for courses taken during the upcoming winter term by December 1st. The absolute deadline for submitting a tuition benefit application to the Benefits Division is December 31st for courses taken during that calendar year.
