Below are various forms for the Retirement plan.

Newly eligible faculty & staff may conveniently elect or waive retirement plan participation by using the eBenefits, HRIS Self Service module. (New Hires Only)
Use this form to authorize tax deferred contributions during calendar year 2008 for Basic (Matched) and/or Supplemental (Unmatched) contributions. Please Note: Supplemental contributions will generally commence the month following the month a completed enrollment forms is received by the Benefits Division or Chicago HR Office. Basic contributions commence the month following the month the 1 year of service and age 24 eligibility requirements are met.
Use this form to waive the one year waiting period for 403b Basic retirement plan participation.
Establishing a TIAA-CREF account
Use this link to access TIAA-CREF's participant web site to establish an account online. Contributions deducted from your paycheck will not be posted until an account is established. For full time faculty the plan ID is NUPLANA. For staff and part time faculty, the Plan ID is NUPLANB.
Establishing a Fidelity account
Use this link to access Fidelity's participant web site to establish an account online. Contributions deducted from your paycheck will not be posted until an account is established. 

(NOTE: Please return the completed applications/forms to the Benefits Division. Up to two Retirement Plan enrollment forms may be completed for a calendar year.)

 
To apply to make early withdrawals from your retirment accounts in times of financial hardship. The reason for the hardship distribution must meet a qualifying IRS reason for the distribution. It also requires the attachment of detailed documentation to support the reason for the distribution. Distributions require the approval of the NU Associate Vice President for Human Resources.
Fidelity Investments Beneficiary Designation
To record or change beneficiary designations for Fidelity Investments accounts. You may do this online.
To authorize another party to trade assets in your Fidelity Investments retirement plan account.
