Faculty and staff may choose single or family coverage and coverage in increments of $10,000 up to a maximum coverage of $1,000,000, not to exceed ten times an individual's annual University salary. The monthly premium rate is $.15 per $10,000 of coverage for single coverage and $.30 per $10,000 of coverage for family coverage. Premiums deducted in the current month on an after-tax basis pay for the next month of coverage.
Example: Employee with an annual University salary of $25,000 chooses single coverage in the amount of $320,000. The maximum available coverage for this individual is $250,000 (10 times the $25,000 annual salary). The monthly premium is $3.75.
The premium rates listed below are for coverage in increments of $10,000 from $10,000 to $300,000 and then in increments of $50,000 to $1,000,000. This section is strictly illustrative and does not mean that coverage above $300,000 is only available in $50,000 increments. Coverage is still available in $10,000 increments above $300,000. For example, an available coverage amount could be $310,000, $420,000, $580,000 or $910,000.
