Changes in Spouse Employment Status


An individual may enroll in the AD&D, health, vision and First Commonwealth dental Plans as a result of a change in a spouse's employment status by submitting a completed enrollment form to the Benefits Division or to the Chicago Campus Human Resources Office within 31 days from the date of the event. Evidence of good health is not required.
