Employees may continue certain coverage during a leave of absence.
Leave of absence with pay
Employees may continue benefit plan participation during a paid leave of absence.
Leaves of absence for health or medical reasons
Employees may continue health, dental and vision insurance coverage
and pay the regular monthly employee premium. The University continues
its contribution.
Leaves of absence for personal reasons
Employees may continue health, dental and vision insurance coverage
and pay the regular total monthly premium.
Coverage continuation procedures
Upon notification of that the leave was approved, the Benefits
Division will forward information to the employee detailing his
or her benefit coverage, the monthly premium rate charged during
the leave and a form for the employee to complete requesting continuation
of benefit coverage. The completed form should be returned to the
Benefits Division. Failure to pay the monthly premium in a timely
manner will result in the termination of coverage.
Return from leave
Upon return from a leave of absence, an individual may resume benefit
coverage not continued during the leave.
An individual who decides not to return from a leave of absence
has the choice of converting life and AD&D insurance coverage
to an individual policy directly with the sponsoring insurance company.
