Changes in Family or Employment Status

Changes in family or employment status can be used as an IRS qualifying event to apply for additional life insurance, modify your FSA elections, change enrollment in disability plans, or even add or drop health insurance. Effective June 13, 2011, proof of dependency is required for all plan beneficiaries being added under medical insurance. A completed Benefits Change Form and supporting documentation must be received by the Benefits Division within 31 days of the event.

 

Changes in Family Status

•  Marriage

•  Birth/Adoption

•  Divorce/Legal Separation

•  Death of Dependent

•  Changes in Partnership

•  Dependent enters/leaves the United States

•  Turning 65 (actively employed)

 

Changes in Employment Status

•  Newly Eligible

•  Termination of Eligibility

•  Position Change

•  Leave of Absence

•  Retirement

•  Change in Spouse Employment Status
