Changes in Family Status |

An individual may change from single to family AD&D, health,
dental and vision coverage as a result of a change in family or
employment status such as marriage, birth or adoption of a child
or a change in a spouse's employment status. Individuals may also
change their family coverage to add coverage for a new family member
as a result of the adoption or birth of a child. An individual may
also enroll in the FSA plans or change his or her FSA annual pledge
amount.
A completed enrollment form must be received by the Benefits Division
or by the Chicago Campus Human Resources Office within 31 days of
the event. Evidence of good health is not required.
An individual may similarly change from family to single coverage
at any time as a result of divorce, death or legal separation. An
individual may drop coverage for a specific family member and continue
family coverage by submitting a completed enrollment form to the
Benefits Division or to the Chicago Campus Human Resources Office.

An individual may drop Dental, Vision and Short Term Disability
coverage as a result of a change in family or employment status.
Completed enrollment forms must be received by the Benefits Division
within 31 days from the date of the event.
An individual may drop AD&D coverage at any time by submitting
a completed enrollment form to the Benefits Division or to the Chicago
Campus Human Resources Office.
