Changes in Employment Status


An individual may enroll in the AD&D, health, dental, vision and FSA plans as a result of a change in employment status. The Benefits Division or the Chicago Campus Human Resources Office must recieve a completed enrollment form within 31 days from the date of the event. Evidence of good health is not required.
