The end of the year is quickly approaching, which means you may need to use up your reimbursement account funds.
• For quick reimbursement, you can submit your claims online at www.healthhub.com
• If you want to fax or mail in a claim, make sure to use the most current claim form. You can find it online in My HealthHub Resources.
• Make sure to sign the claim form.
• If you use the online claim tool and select the Fax option, make sure to sign the fax coversheet. You must send this in with your supporting documentation.
• Do not highlight any part of the form or documentation. Highlighting makes faxed copies hard to read. If PayFlex can't read the documentation, they will deny your claim.
• Always include supporting documentation with your claim. Keep copies for your records.
• The best documentation is the Explanation of Benefits (EOB). Your insurance company will send you an EOB when they process your medical claim.
• If you don't have an EOB, you can send an itemized statement or receipt. It must include the date of purchase or service; the amount you had to pay; a description of the product or service; the name of merchant or provider; and the name of the patient.
• PayFlex cannot accept a receipt or statement showing only the amount paid, balance due or that insurance is pending or filed.
• PayFlex cannot accept a debit or credit card receipt or a cancelled check as documentation.
• You must first receive the dependent care services before you can receive reimbursement. This is true even if your provider requires payment before you receive the care.
• If you send in a claim before the service end date, the claim will be placed in a "Hold" status. You will not receive an Explanation of Payment (EOP) at this point but will see the status online. Once the service date has passed, your claim will be released from the hold status. For example: On March 5, you send in a claim for March 1 through March 14. We'll hold that claim until March 15.
• When you submit a claim online, you must now enter the name and age of the person in care.
• If you do not have documentation for dependent care expenses, your provider can print their business or individual name AND sign the claim form.
• We cannot reimburse fees for activities, supplies, field trips, meals and diapers when listed separately.
