We encourage you to provide us feedback about the quality of our service with our Web Comment Card. Additionally, many patients receive an invitation via e-mail from StudentVoice.com to complete a Web-based Patient Satisfaction Inventory. The staff of the Health Service are committed to providing the Northwestern community a high degree of service and our comment card system is one mechanism by which you may communicate with us.

You may choose to be anonymous if you wish, but for those who provide us with a name and an address or phone number, we may be following up directly with you to help ensure your satisfaction with the Health Service. Please do not use the Web Comment Card for any request for medical care or to schedule an appointment. All comment cards are reviewed by the Executive Director of the Health Service and the members of the Health Service Executive Committee. Comment cards and the various patient satisfaction surveys the Health Service utilizes are an important part of our quality management program and we encourage you to be part of this communication process. Remember, improvement comes only with communication and suggestions.
