Student Immunization Form Requests
To have forms completed to verify your immunization status for an away rotation or other program:
- Submit the form along with a copy of your immunization record to the Northwestern Medicine Student Health Service nurse, Lynn Carter, LPN. Forms may be dropped off at the Galter 18th floor appointment desk or faxed to 312.695.2857.
If you need to obtain copies of your immunization record:
- Current students may access immunization records through the Northwestern Medicine Student Health Service Evanston NU Personal Health Portal (PnC).
- Or, complete the Authorization for Release of Confidential Information in order to request that your record be sent directly to Nurse Carter by Health Information Management Services at the Northwestern Medicine Student Health Service/Evanston.
