Northwestern University Financial Systems Forms

The following forms are available to users of EP²S. In order to view (and print) several of the forms found on this site, you will need to have the Adobe Acrobat Reader version 3.0 or above. In most cases, this program should already be installed on your computer. If you do not have Adobe Acrobat Reader, it can be found at Adobe Acrobat Reader. This is a free program and can be downloaded from this site.

CUFS2000 Access Request Form PDF Format - Using Acrobat Reader 3.0 or above. Security forms for new users requesting access to the CUFS2000 System. This form can be filled out and printed. Note that original signatures are required.

90-Day Rule Justification Form - (Word Document)
90-Day Rule Justification Form (PDF) Form to be used to accompany any transaction over 90 days
T & E Reimbursement Forms Including the T & E Report Form, Visitor's T & E Report Form and T & E Policy Exception Form. The T & E form is used for any travel and/or entertainment, including lunches for guests on campus.

Non-Travel Expense Report Form (Microsoft Excel Format) Reimbursement form for purchases of supplies not relating to travel. This form is not to be used for entertainment.
