Two statements packages are provided at no charge for each chapter - additional individuals
will be charged $10 each for a complete packet.

Reprints:

Current fiscal year: $10 for one monthly CUFS statement and additional statements same month @ $3 each

Prior fiscal years: $25 for one monthly CUFS statement and additional statements @ $5 each
